By signing this document I have indicated permission for Planet Eclipse LLC to obtain credit information from the sources I have referenced, including external credit reporting sources and any consumer credit agency. I, the undersigned, acknowledge and consent that failure to pay as agreed may result in the said business incurring attorney fees, collection fees and court costs and that said account shall be terminated.

Applicants in the USA are subject to a MAP (Minimum Advertised Pricing) policy that must be followed at all times. If MAP pricing is violated then Planet Eclipse LLC has the right to put my account on hold at their discretion.
